At Home 2 Care, LLC, we provide safe, respectful housing for adults in Cleveland. Since 2015, our warm, person-centered homes have helped individuals feel secure, maintain independence, build daily living skills, and stay connected to a stable, supportive community network.

How It Works

We start with a simple phone call or online inquiry, then we review basic eligibility, answer questions, and schedule a tour. Together, we design a personalized support plan, coordinate benefits and referrals, and set a move-in date. Our team guides residents and partners through each step, stays in close contact during the transition, and checks in regularly to adjust support as needs and goals change.
